HVC Site Direction Questionnaire:


1. Highway Access:

   - What is the specific exit off the highway to reach the HVC outpatient center?

Take I-81 N, I-78 E, I-287 N, I-87 S and I-95 N to Seaview Ave in Bridgeport. Take exit 29 from I-95 N

Follow CT-8 S to CT-127 S/White Plains Rd in Trumbull. Take exit 4 from CT-8 S

   - Are there any notable landmarks or signs to look out for? Boston ave. to Grant st.

 

2.  Parking Information: 

   - Is there designated garage parking for patients? 

Main parking garage (KAULBACH)- across from the main entrance by water fountain.

   - If yes, which level of the garage is recommended for HVC visitors? Any available space.  Levels 1 & 3 recommended (all parking is free)

   - Are there assigned parking spaces for patients? Yes, marked “patient parking”

 

3.  Building Entrance: 

   - Is there a specific entrance to the building for HVC visitors? Main entrance

   - Are there any distinctive features or signage to identify the entrance easily? Yes

 

4.  Elevator Directions: 

   - Upon entering the building, which elevator or set of elevators leads to the HVC outpatient center?Walk through the main lobby, pass through security and stop at admitting (unless instructed otherwise)  Proceed to the main elevators and go to the 10th floor.

   - When exiting the elevator, which direction should patients follow to reach the HVC? 

Glass doors directly across from elevators on the 10th floor

 

5.  Floor Information: 

   - On which floor is the HVC outpatient center located? 10

 

6.  Registration Process: 

   - Where is the registration area for HVC appointments? 1st floor, admitting 

(Transesophageal echocardiograms are pre-registered and may proceed directly to the 10th floor after passing through security)

   - Is there an automated check-in machine, and if so, where is it located? No

 

7.  Additional Landmarks: 

   - Are there any specific landmarks or points of reference inside the building that would assist patients in finding the HVC? 

1st floor lobby, take hallway past gift shop on left and admitting on right to the main elevators

 

8.  Accessibility Features: 

   - Are there any accessibility considerations or special routes for patients with mobility challenges? No,  parking and entrance to the hospital are fully accessible.



9.  Noteworthy Instructions:   

   - Are there any unique or noteworthy instructions that patients should be aware of when navigating to the HVC outpatient center? Patients should anticipate passing through a metal detector at security upon arrival prior to registration.
