HVC Site Direction Questionnaire:

1. Highway Access:

   - What is the specific exit off the highway to reach the HVC outpatient center? (from CT 8 South) Exit 7 from CT 8 North) Exit 5

   - Are there any notable landmarks or signs to look out for? AMC Movie Theater

2.  Parking Information: 

   - Is there designated garage parking for patients? No

   - If yes, which level of the garage is recommended for HVC visitors? NA

   - Are there assigned parking spaces for patients? Yes (parking lot)

3.  Building Entrance: 

   - Is there a specific entrance to the building for HVC visitors? No- general entrance

   - Are there any distinctive features or signage to identify the entrance easily? Large sign outside the cardiology suite

4.  Elevator Directions: 

   - Upon entering the building, which elevator or set of elevators leads to the HVC outpatient center? From the front entrance straight then left turn.  From back entrance straight then right turn.

   - When exiting the elevator, which direction should patients follow to reach the HVC? Turn right off the elevator.

5.  Floor Information: 

   - On which floor is the HVC outpatient center located? 4th.

6.  Registration Process: 

   - Where is the registration area for HVC appointments? In the front of the suite.

   - Is there an automated check-in machine, and if so, where is it located? No.

7.  Additional Landmarks: 

   - Are there any specific landmarks or points of reference inside the building that would assist patients in finding the HVC? Large sign right outside the suite

8.  Accessibility Features: 

   - Are there any accessibility considerations or special routes for patients with mobility challenges? Wheelchairs in the lobby on the main floor.
