The Digital Access Program by Rx.Health is here to help you take advantage of government benefits like a free phone and free service.

If you are interested in this program, please enroll. You can do so by

doing the following.

1. Click this link: https://survey.covered.health/s3/Digital-Access-Program-Enrollment.
2. Enter your name, phone number, and email. Click submit.
3. You will receive a short questionnaire to fill out. You will not need any additional

information for this questionnaire.

Eligibility
Individuals who enroll in the Digital Access Program will receive guidance regarding digital access. You may receive a free phone and service from the ACP based on eligibility.
