  1. How do patients transition from control period to interventional period?
    • Once patients are enrolled in the study, they will enter the study's 12-week control period. Patients are monitored with PRO2/3 and education content once every two weeks during this phase. Week 12's final engagement includes an introduction to the Health PROMISE app and email/SMS-based monitoring.
    • All messaging will be automated.
    • No Care coordinated intervention is required
  2. Where will patient engagement data from the Health PROMISE app and email/SMS monitoring be viewed?
    • All data will be populated into RXUniverse's existing patient level custom dashboard.
  3. What happens when patients use both email/text monitoring and the Health PROMISE app?
    • All data will be populated into RXUniverse's existing patient level custom dashboard under the specific patient regardless of which monitoring platform they use.
  4. What is the eligibility requirement for the monthly $10 gift card? (Email/sms based monitoring and HP app)
    • Patients who have completed the e-PRO twice or more during the month are eligible.
    • The gift cards will be sent/distributed at random from among all eligible patients by the care coordinator.

  5. What are the criteria for care coordinators to use when sending nudges to patients during interventional period?( Email/sms based monitoring and HP app)
    • If a patient does not enter data for three weeks, an alert will be generated on the dashboard.
    • If the patient does not enter data for three weeks, care coordinators should begin nudge with two SMS messages, two phone calls, and a chat with the patient during their next appointment.
    • If the patient does not have an appointment, an additional two phone calls must be made. 
    • If the patient does not respond after one month of nudging, he or she will be dropped out from the study.
  6. What happens to the patients that are enrolled but not responding?
