- How do patients transition from control period to interventional period?
- Once patients are enrolled in the study, they will enter the study's 12-week control period. Patients are monitored with PRO2/3 and education content once every two weeks during this phase. Week 12's final engagement includes an introduction to the Health PROMISE app and email/SMS-based monitoring.

- All data will be populated into RXUniverse's existing patient level custom dashboard under the specific patient regardless of which monitoring platform they use.
- What is the eligibility requirement for the monthly $10 gift card? (Email/sms based monitoring and HP app)
- Patients who have completed the e-PRO twice or more during the month are eligible.
- The gift cards will be sent/distributed at random from among all eligible patients by the care coordinator.

- What are the criteria for care coordinators to use when sending nudges to patients during interventional period?( Email/sms based monitoring and HP app)
- If a patient does not enter data for three weeks, an alert will be generated on the dashboard.
- If the patient does not enter data for three weeks, care coordinators should begin nudge with two SMS messages, two phone calls, and a chat with the patient during their next appointment.
- If the patient does not have an appointment, an additional two phone calls must be made.
- If the patient does not respond after one month of nudging, he or she will be dropped out from the study.
