This report must be sent to WHO within 48 hours of identifying a case with COVID-19.
This report has 4 sections
1. Patient information
2. Clinical status
3. Exposure risk in the 14 days prior to symptom onset
4. Outcome: This section is completed and resent along with full form as soon as the outcome of the disease is known or after 30 days after the initial report.
