Information you will need to apply as an HCP site:
- Site or consortium information
- The FCC Form Application number and the HCP number will already be filled in
- If we are applying for you as a consortium, enter our FCCRN in line Rx.Health’s FCCRN is: 0029423944
- If you are applying as an individual HCP, enter the FCCRN you obtained from Step 1 of this cheat sheet.
- Non-Profit Tax ID (EIN)
- Eligibility Category
- You must fall into one of the following eligibility categories:
- Contact information of primary account holder/ project coordinator
- NPI, Organization Taxonomy Code (registry search provided on form):
- Supporting Documentation
- Examples may include: Hospital licenses, with the address clearly posted, to verify a non-profit hospital
- A recent bill or lease, with the address clearly posted, to verify the HCP address provided on the FCC Form 460
- Public tax documents to verify the Non-Profit Tax ID (EIN)
- Health Resources and Services Administration (HRSA) accreditation to verify a community health center
If you are applying as a Consortium yourself, the process is similar to above except:
- For Consortium Information, you will need to enter the consortium FCCRN obtained in CORES from Step 1
- For Eligibility Category, Consortium of the above will already be preselected
- For Supporting Documentation, an example that could be included with the FCC Form 460 is recent bill or lease, with the address clearly posted, to verify the address provided.
- For Consortium Leader Information, you will need to have a written agreement allocating legal and financial responsibilities and upload it
